Deaths of Children in Out-of-Home Care: A System That Keeps Failing Children

Children have died in out-of-home care and shortly after leaving it while still known to the system. This register brings individual accounts together with published reporting on warning signs, disrupted placements, supervision and accountability. Supporting articles below provide the wider context; they are not additional child-death entries.

Governments Have Been Told, Repeatedly

The Australian Institute of Family Studies and Australian Human Rights Commission examined more than 3,000 recommendations from 61 child-protection and youth-justice reports and inquiries between 2010 and 2022. The recurring problems included weak oversight, poor information sharing, workforce limitations, inadequate investment, limited First Nations partnership and children being denied a meaningful voice. 1 2 3

Alecomm’s Conclusion: The repeated diagnosis is itself damning. Governments have a substantial body of evidence explaining what is failing. An announced review is not proof that a child is safer. Accountability requires showing which recommendations were implemented, what changed and who remains responsible when the same failure returns.

Repeated Moves Can Strip Away A Child’s Support

The register includes reporting on Child RK’s 70 care arrangements, RM’s approximately 57 placements and Zhane Chilcott’s 18 placements. The Zhane reporting describes coronial findings about failures that increased his suicide risk, including the loss of a stable placement after additional support funding was refused. 4 5 6

Alecomm’s Conclusion: A move changes far more than a file’s address. It can disrupt trusted adults, school, treatment, family contact and cultural connection. These cases make placement continuity and the support needed to sustain it urgent matters of care. The records do not establish that every placement move caused a death; they expose why repeated disruption needs a named decision-maker, reasons and scrutiny.

Money Can Flow While Care Fails

The NSW Government’s account of the Talbot–Dean review describes a $2 billion outsourced out-of-home care system with a profound lack of accountability, ineffective oversight and fragmented responses. Reviewers were often unable to establish whether funding meaningfully supported children. The review called for financial transparency, identifiable direct services, performance measures and audits. 7 8 9

Alecomm’s Conclusion: A large invoice is not evidence of a safe placement. Funding must be traceable to the support a child actually receives. The failure to follow the money is an accountability failure in its own right and creates room for waste and exploitation. Tracing those transactions and identifying who authorised them is essential to establish the responsibility of particular providers and officials.

Suicide And Mental Health Response

These case accounts record suicide, self-harm or mental health concerns. The linked entries preserve what was reported and any official findings. 10

Alecomm’s Conclusion: A child’s distress requires timely, sustained support and a clear account of who was responsible for responding.
